Our PDF requires you to have Adobe (Acrobat) Reader version 7. [ Download it HERE ]. Alternatively, you can email the completed form (MS Word version) to Brad Stephens as directed at the bottom of the form or print either version for leaving in our Student Government Mailbox.

** The van request form can be filled out in microsoft word and then emailed back to us at ucnightwalk [at] gmail.com. It can also be printed out, completed by hand in ink and delivered to the NightWalk mailbox in the Student Government office. If you decide to drop off the form in the office, please email us to let us know it's waiting to be reviewed.
